How to fill in Form LP1F / LP1H
Pick the right form
Use LP1F for property and financial affairs or LP1H for health and care decisions. The LP1H is on the same GOV.UK page. If you want both, complete both forms.
Fill in the donor and attorney details
Enter the donor's details, then each attorney and any replacement attorneys, and how they should make decisions together. Set the printer page size to A4 or Fit before you print.
Add instructions and the certificate provider
Add any preferences or instructions, and name a certificate provider aged 18 or over who confirms you are making the LPA by choice.
Print and sign by hand in the right order
The donor signs first in front of a witness, then the certificate provider signs, then the attorneys and replacement attorneys sign in front of a witness. Everyone signs the same original paper form.
Register the LPA
Send the original signed form with the £92 fee to Office of the Public Guardian, PO Box 16185, Birmingham, B2 2WH. Registration takes 8 to 10 weeks if there are no mistakes.
