How to fill in Form MC1
Read and sign the declaration (Part 1)
Part 1 contains the declarations and consents. It must be signed and dated; if you cannot sign, you can make a mark.
Your details and dependants (Parts 2 to 4)
Enter your personal details and those of your spouse or partner, your dependent children, and any EU or UK social security details.
Income and expenses (Parts 5 and 6)
Give income details for everyone in the application and household expenses such as rent, mortgage, childcare and travel to work. Tick each 'Evidence enclosed' box as you gather the documents.
Get your GP to complete Part 7
Your GP must hold an HSE contract and sign, date and stamp Part 7 with their GMS number. If your spouse or partner uses a different GP, that GP completes Part 7A.
Assisted decision making (Part 8), if needed
Complete Part 8 only if you are applying for someone else under a decision-making arrangement.
