Modelo 145 is the form an employee in Spain gives to their employer (or other payer) to report personal and family circumstances, so that the right amount of IRPF income tax is withheld from pay. It goes to the payer, not to the Agencia Tributaria. Open the official form from agenciatributaria.gob.es in BunnyDoc's free Sign PDF tool straight from this page, type into its fillable fields, add your signature and date, and download it. Everything happens in your browser, so the file never leaves your device.
How to fill in and sign Modelo 145
Section 1: your details
Enter your NIF, name and year of birth, your family situation, any disability and whether you moved for a job (movilidad geográfica).
Section 2: children and descendants
List children and other descendants under 25 (or older with a disability) who live with you.
Section 3: ascendants
List parents or grandparents over 65 (or younger with a disability) who live with you.
Sections 4 and 5: payments
Report court-ordered compensatory pensions or child maintenance, and, if it applies to you, payments for your main home financed with a loan.
Section 6: sign and date
Sign and date the declaration. You can choose not to give some data, but the form warns that your withholding may then be higher than needed.
Give it to your employer
Hand it to the company or payer, who signs the acknowledgment of receipt (section 7). The form has one copy for the payer and one for you. Do not send it to the AEAT.

Common mistakes
- Sending the form to the Agencia Tributaria instead of the employer.

Modelo 145: frequently asked questions
Who do I give Modelo 145 to?
To your employer or other payer of your salary. It is not filed with the Agencia Tributaria.
Can Modelo 145 be filed online with the AEAT?
No. The AEAT says the features of this declaration do not allow electronic filing with it, because the form is given to the payer.
What if I don't hand in a Modelo 145?
