Print and sign by hand
The annexures are signed on paper and produced with your application documents. The Passport Seva instruction booklet says to use only a blue or black ballpoint pen for signatures.
Passport Seva annexures are specimen formats for declarations and certificates that you add to an Indian passport application in particular cases, for example Annexure D when both parents consent to a minor's passport, or Annexure F for a lost or damaged passport. Open the official annexures from passportindia.gov.in in BunnyDoc's free Sign PDF tool, type in the text of the annexure you need and download it to print and sign by hand. Everything happens in your browser, so the file never leaves your device.
How to fill in Passport annexures
Find the annexure you need
Check the document list for your application type. Common ones are Annexure C (one parent has not given consent for a minor), D (both parents' declaration for a minor), E (applicant's declaration), F (lost or damaged passport), G (no objection certificate) and H (prior intimation by a government employee).
Use the specimen wording
Copy the specimen exactly and fill in the details it asks for, such as the minor's name and date of birth, your passport or ID number and the application reference number (ARN).
Use the right paper or letterhead
Annexures C, D, E, H and I are self-declarations on plain paper. Annexure A is issued on the employer's original stationery and Annexure B on the official letterhead of the verifying authority.
Sign by hand
Print the annexure and sign it with a blue or black ballpoint pen. Annexure D is signed by both parents or the legal guardian; Annexures G and B are signed and stamped by the issuing officer.
Take it to your appointment
Submit the signed annexure with your other application documents when you attend your appointment, following the document list for your application type.
What you need before you start
- Your application reference number (ARN), once you have applied online.
- The passport, Aadhaar or voter ID numbers that the annexure asks for.
- For a minor: details of both parents or the legal guardian, and a copy of the court order if a guardian signs.
- The annexure document from passportindia.gov.in.
Do annexures need a notary or stamp paper?
The revised annexures mark the parents' and applicant's declarations (Annexures C, D, E and I) as self-declarations on plain paper, and do not ask for notarisation or stamp paper. Annexure G, a no objection certificate, is valid for six months from its date of issue.
Common mistakes
- Changing the specimen wording.
- Only one parent signing Annexure D.
- Signing with anything other than a blue or black ballpoint pen.
- Bringing an Annexure G that is more than six months old.
