How to fill in Schengen visa form

  1. Identity and travel document (fields 1 to 16)

Enter your surname, birth surname, first names, date and place of birth, nationality and sex, then your passport type, number, issue and expiry dates and issuing country. Field 10 is for a minor's parent or guardian.

  2. EU family member (fields 17 and 18)

Only complete these if you are a family member of an EU, EEA or Swiss citizen. If so, you skip some later fields marked with an asterisk.

  3. Occupation and journey (fields 21 to 27)

Give your occupation and employer, the purpose of the journey, your main destination, first entry country, number of entries and planned arrival and departure dates.

  4. Host and costs (fields 30 to 32)

Name the inviting person, hotel or company, and say who pays for your trip and which means of support you have.

  5. Who filled in the form (field 33)

Only complete this if someone else filled in the form for you.

  6. Sign and lodge it at the consulate

Print the form and sign the declaration with place and date. Lodge it in person with your passport, photo, travel insurance and supporting documents at the consulate or visa centre, where your fingerprints are taken.
