Landlord details
Enter the owner's, landlord's or agent's name or company name, address, contact details and SIRET number if any.
Tenant and housing
Give the tenant's or tenants' names and move-in date, the address, whether it is a room, the living area and whether it is a flat share.
Rent
Enter the monthly rent excluding charges and the charges for the month of entry and for July, or the total for a flat share. Say whether the tenant is up to date with the rent.
Conventionné housing and direct payment
Fill in the convention codes if the housing is conventionné, and say whether you want the aid paid directly to you (first application only).
Decency, signature and sending
Confirm whether the housing meets the decency criteria, then sign, stamp and date the certificate. Give it to the tenant to send with their housing aid claim, unless the Caf or MSA tells you otherwise.
