Cerfa 14952 is the French proxy voting form. It lets another registered voter (the mandataire) vote for you. Open the official form from formulaires.service-public.gouv.fr in BunnyDoc's free Sign PDF tool straight from this page, type your details into its fillable fields and download it to print. You then sign it by hand in front of a police officer, gendarme, court clerk or consular official. Everything happens in your browser, so the file never leaves your device.

How to fill in Cerfa 14952

  1. Get both voter numbers

You need your own numéro national d'électeur and that of your proxy, plus their name and date of birth. Your number is on your voter card or on the Service-Public.fr page "Interroger sa situation électorale".

  2. Fill in the proxy section

Type your details and your proxy's details into the "Vote par procuration" part and the "Récépissé à conserver par le demandeur" receipt.

  3. Choose how long the proxy lasts

Choose the first round, the second round, both rounds, or a period of up to 1 year (up to 3 years for voters on a consular list).

  4. Print on two separate sheets

The notice says to print the form on two separate sheets, not double-sided, so that the receipt can be handed back to you.

  5. Sign in person in front of the authority

Take the form and an original ID document to any police station or gendarmerie, the tribunal judiciaire or tribunal de proximité where you live or work, or a consulate if you live abroad. You sign there and keep the receipt.

What you need before you start

  • An original identity document such as an ID card, passport or driving licence. Your voter card is not an ID document.
  • Your national voter number and your proxy's.
  • Your proxy's surname, first names and date of birth.

Can I sign the proxy form electronically?

No. The official notice says you must sign personally in front of the authority, which checks and photocopies your ID. You can fill in the form on screen, but the signature is made by hand at the counter.

The alternative is the online Maprocuration service. You log in with FranceConnect, then either go in person to have your identity checked or certify it online with France Identité, as explained on Service-Public.fr.

Rules for the proxy

  • Your proxy must be registered on an electoral list, but does not need to live in your commune. They vote at your polling station.
  • A voter registered in France can hold two proxies, of which at most one made in France. A voter on a consular list can hold three.
  • You can cancel a proxy at any time through the same authorities.
  • Apply as early as possible. A proxy that reaches the town hall late may not be usable.
