The Composite Declaration Form (Form-11) is the declaration a new employee gives their employer about any previous Employees' Provident Fund (EPF) and pension membership, their Universal Account Number (UAN) and their KYC details. The employer uses it to link you to your UAN and, where your KYC is verified, to transfer your old PF balance automatically. Download the form from your employer or the EPFO website, open it in BunnyDoc's free Sign PDF tool, type into its fields, add your signature and date, and download it. Everything happens in your browser, so the file never leaves your device.

How to fill in and sign EPF Form 11

  1. Get the form

Your employer will usually give you Form-11 when you join. It is also published by the EPFO.

  2. Your details (items 1 to 6)

Enter your name, father's or husband's name, date of birth, gender, marital status, email and mobile number.

  3. Joining date and KYC (items 7 and 8)

Give your date of joining, your bank account number and IFSC code, Aadhaar number and PAN if you have one.

  4. Previous membership (items 9 to 12)

Say whether you were a member of the EPF Scheme, 1952 or the Employees' Pension Scheme, 1995. If so, give your UAN or old PF account number, the previous establishment, dates of joining and exit, and any scheme certificate or PPO number.

  5. International workers (item 13)

If you are an international worker, give your country of origin and passport number and validity.

  6. Sign the undertaking and hand it in

Sign and date the undertaking, which authorises the EPFO to use your Aadhaar for verification and asks for any old PF account to be transferred. Give the form to your employer, who completes and signs the employer's declaration and keeps the form.

What you need before you start

  • Your UAN and previous PF account number, if you have worked in an EPF-covered job before.
  • Dates of joining and leaving your previous job.
  • Your Aadhaar, PAN and bank account number with IFSC code.
  • Your passport details, if you are an international worker.

What the employer does with it

The EPFO's information booklet for employers tells employers to obtain the Form 11 self-declaration from each member with copies of their KYC documents, and to keep it for future reference. The employer's part of the form records your PF number and UAN and whether your KYC has been approved, which decides whether an old account can be transferred automatically.

Common mistakes

  • Leaving out your previous UAN or PF account number when you had one.
  • Wrong dates of joining or leaving the previous job.
  • Forgetting to sign the undertaking.
