How to fill in and sign Form IMM 5476
Open the current form and choose what you are doing
Open the November 2025 edition from canada.ca. At the top, tick whether you are appointing a representative, updating their contact details, cancelling one, cancelling and appointing a new one, or withdrawing as a representative. Each option tells you which sections to fill in.
Section A: applicant information
Enter the applicant's name, date of birth and contact details, the type of application and its number if it has already been submitted, and the IRCC ID or UCI if known.
Section B: appointment of representative
Give the representative's full name, say whether they are paid or unpaid and, for a paid representative, their membership ID with their regulatory body. The representative signs and dates the declaration in this section.
Section C or D, if they apply
Use Section C to name a representative you are cancelling. Section D is for a representative who is withdrawing; they sign it there if the applicant's signature cannot be obtained.
Section E: applicant declaration
The applicant signs and dates Section E. Electronic signatures are accepted when the form goes through a portal or secure account; outside those, sign by hand.
Submit the form
Upload it with your online application, or include it with a paper application. If your application is already submitted, use IRCC's web form or mail it to the office processing your application.

Who can be a paid representative?
IRCC deals only with paid representatives in good standing with their regulatory body: members of the College of Immigration and Citizenship Consultants (CICC), lawyers and paralegals who are members of a Canadian provincial or territorial law society, notaries who are members of the Chambre des notaires du Québec, and students-at-law under their supervision. IRCC says an application using a paid representative who is not authorised will be returned. Unpaid representatives, such as a friend or relative, do not need a membership ID.
Can I sign IMM 5476 electronically?
Yes, when the form is submitted through an IRCC portal or secure account. The IRCC form page lists a typed name, signing software, a scanned or image signature and a checked box as accepted. Outside a portal or secure account, IRCC requires a handwritten signature, although you may send a scan of the hand-signed form. A thumbprint is accepted for someone who cannot read or cannot sign for a physical reason. For background, see what an electronic signature is.
