How to fill in and sign Form IMM 5707
Open the form on a computer
IRCC asks you to complete the form on a computer and validate it electronically. A handwritten form cannot be validated.
Section A: you, your spouse or partner and your parents
Give your own details, then those of your spouse or common-law partner and both parents, including dates of birth, countries of birth, addresses and occupations, and whether each person will come to Canada with you.
Section B: your children
List all your children, including adopted children and stepchildren. If you have none, follow the form's instruction for applicants with no children.
Section C: your brothers and sisters
List your brothers and sisters, including half and step siblings.
Sign, validate and submit
Sign and date the declaration in Section C, use the Validate button, and include the form with your visa or permit application.
What you need before you start
- Full names, dates and countries of birth of your spouse or partner, parents, children and siblings.
- Their current addresses and occupations, and dates of death where relevant.
- Your marital status history.
Can I sign IMM 5707 electronically?
The IRCC form page says to sign and date the declaration in Section C, but does not say whether an electronic signature is accepted. For online applications, IRCC's help centre explains how the application itself is signed electronically when you submit. If you are unsure, follow the instructions for your application type.
Common mistakes
- Leaving out family members, such as a child from a previous relationship.
- Leaving a section blank instead of following the form's instruction for having no spouse or children.
- Filling it in by hand, which means it cannot be validated.
- Only one family form for two adult applicants.
