How to fill in KVK Form 1

  1. Start or continue (section 1)

Say whether you are starting a new eenmanszaak or continuing an existing business or establishment, and from which date.

  2. Details of the eenmanszaak (section 2)

Enter trade names, a description of the activities, sales channels, import or export, addresses, contact details and the number of people working in the business. Free-text answers need a Dutch translation.

  3. Owner details (section 3)

Give the owner's personal details and add a handwritten signature in 3.1, not a copy or scan.

  4. Advertising and hiring out staff (sections 4 and 5)

Choose whether you want to receive less unsolicited advertising, and say whether you hire out employees.

  5. Sign the form (section 6)

Print the full set, including the Dutch version, and sign by hand in section 6.1 to confirm the details are correct. Cross out mistakes and initial each correction.

  6. Make an appointment or post it (section 7)

If you take over a business not yet registered with KVK, bring the form to a KVK appointment. If you live on the Wadden Islands or take over a business already registered, you can post it with a copy of your ID.

What you need before you start

  • A valid ID document (bring the original to an appointment, or a copy by post).
  • Proof of the start date if it was more than 2 months ago, such as a purchase invoice.
  • A franchise contract if you are a franchisee.
  • Form 9 for each extra establishment and Form 13 for authorised representatives, if needed.
  • Proof you may use the business address if it differs from your home address.

Fee and how to submit

KVK charges a one-off registration fee of €85.15 in 2026 for a new registration, and sends the invoice by post when you register on paper. Book appointments on kvk.nl and find postal addresses on the KVK postal addresses page.

Common mistakes

  • Signing only the English pages instead of the Dutch version.
  • Sending a scanned or copied signature.
  • Leaving business activities untranslated.
  • Forgetting the copy of the signer's ID when posting the form.
