KVK Form 14 changes the details of a business or establishment, including transferring or ending an eenmanszaak, and Form 16 changes or deregisters officials such as directors, partners and authorised persons in the Dutch Business Register. Small businesses use them for changes that cannot be made in My KVK. Download the right form from kvk.nl, open it in BunnyDoc's free Sign PDF tool, type your details into its fillable fields and download it to print and sign by hand. The PDF is handled in your browser and never leaves your device.

How to fill in KVK Forms 14 / 16

  1. Check whether you can do it online

Most business changes can be made online in My KVK. You need Form 14 for changes such as transferring an eenmanszaak or establishment, which cannot be done online.

  2. Pick the right form

Use Form 14 for business or establishment details (address, activities, trade names, transfer or end). Use Form 16 for changes to officials: partners, sole shareholders, directors, supervisory directors and authorised persons.

  3. Fill in the English form on screen

Download the form from its KVK page and complete it. The attached Dutch version fills in automatically. Translate any free-text answers into Dutch.

  4. Print and sign the Dutch version

Print the whole set and sign the Dutch version by hand.

  5. Send it to KVK with ID

Post it with a copy of the signer's valid ID (photo made unrecognisable), or bring it to an appointment if you already have one. For a transfer, the new owner must provide the original form, not a copy or scan.

What you need before you start

  • The KVK number and establishment number.
  • The new details and the date of the change.
  • For Form 16, the personal details of each official being added, changed or removed.
  • A copy of the signer's ID.
  • The form from Form 14 or Form 16 page.

Common mistakes

  • Using a paper form for a change you could make online in My KVK.
  • Signing the English pages instead of the Dutch version.
  • Sending a scan or copy of the form for a transfer, when the original is required.
  • Leaving the photo visible on the ID copy.
