How to fill in and sign Form C-234
Section 1: personal data
Enter your details as the applicant (biological mother or other parent), the other parent only where the form asks for it, and any legal or voluntary representative.
Section 2: reason for the claim
Choose birth or surrogacy, with the date and number of children, or adoption, guardianship, foster care or tutela, with the date of the decision.
Sections 3 and 4: rules and leave dates
Say whether the Estatuto de los Trabajadores or the public employee statute (EBEP) applies, then give the start and end dates of your leave. A biological mother can say if leave starts before the birth.
Sections 5 to 8: allowance, tax, bank and consent
Answer the multiple-birth allowance question, give your tax province, your bank IBAN in your own name, and whether you agree to electronic processing.
Sign and submit
Online through the Seguridad Social electronic office, no signature is needed. Otherwise sign it and send it by post, or take it to a CAISS office with an appointment.
What you need before you start
- ID (DNI, NIE, TIE or passport) for you and any representative.
- The employer's certificate, usually sent by the employer through the RED system.
- The libro de familia or birth registration certificate if it is not already in the Civil Registry's automated records.
- The maternity report from the public health service if leave starts before the birth.
- For adoption or foster care, the court or administrative decision.

Processing time and payment
The Seguridad Social says the provincial director decides within 30 days of receiving the application. The benefit is 100% of the regulatory base and is exempt from income tax, except in Navarre. The claim lapses 5 years after the event, and payment can be backdated up to 3 months before the application.
