The form has a signature and date in the fund section you complete. The ATO does not say whether an electronic signature is accepted; it says you can complete the form electronically or print it, and you return it to your employer, not the ATO.

The Superannuation (super) standard choice form (NAT 13080) tells your employer which super fund to pay your super guarantee contributions into. Employers give it to new employees, and you can use it later to change funds. Download the form from ato.gov.au, open it in BunnyDoc's free Sign PDF tool, type into its fillable fields, add your signature and date, and download it. Everything happens in your browser, so the file never leaves your device.

How to fill in and sign Form NAT 13080

  1. Get the form from your employer or the ATO

Your employer should give you the form with Section C already filled in with their default fund. You can also download it from ato.gov.au.

  2. Section A: your details

Every employee fills in Section A with their full name, employee number if known and tax file number, and ticks which kind of fund they are choosing.

  3. Choose one fund section

Complete one more section only: Section B for an existing fund such as a retail or industry fund, Section C to use your employer's default fund, or Section D for a self-managed super fund (SMSF).

  4. Attach any documents and sign the section

Section B needs a letter of compliance from your fund. Section D needs a document confirming the SMSF is an ATO-regulated fund, which you can check at superfundlookup.gov.au. Sign and date the section you completed.

  5. Give it to your employer

Return the completed form to your employer. Do not send it to the ATO or to your super fund. Your employer has up to two months to act on your choice.

What you need before you start

  • Your tax file number.
  • For an existing fund: the fund's name, ABN and unique superannuation identifier (USI), your member account number and a letter of compliance from the fund.
  • For an SMSF: its name, ABN, electronic service address, bank account details and a document showing it is ATO-regulated.
  • The current form from ato.gov.au.

What your employer does with it

The ATO says employers have up to two months to act on a completed form, and must still pay contributions by their due date. Employers keep completed forms for five years. The ATO also says not to send choice forms to the ATO, and employers should not send them to super funds.

Common mistakes

  • Completing more than one of Sections B, C and D.
  • Leaving out Section A.
  • Sending the form to the ATO or to the fund instead of the employer.
  • Wrong fund details, such as an old USI or member number.
