Form DS-11 is the application for a US passport for people who must apply in person: first-time applicants, all children under 16, and anyone whose last passport was issued before age 16, more than 15 years ago, or was lost, stolen or damaged. Open the official form from state.gov in BunnyDoc's free Sign PDF tool straight from this page, type your details into its fillable fields and download it to print. Do not sign it until the acceptance agent asks you to. The file is handled in your browser and never leaves your device.

How to fill in Form DS-11

  1. Check that DS-11 is the right form

If you are an adult renewing a passport that was issued after age 16 within the last 15 years and is undamaged, you may be able to renew by mail with Form DS-82 instead.

  2. Open the current form

Use the button on this page to open the DS-11 PDF from eforms.state.gov in Sign PDF.

  3. Fill in your details

Type your name, date and place of birth, SSN, contact details, physical description, parents' details and emergency contact. Choose a passport book, a passport card or both.

  4. Print it, unsigned

Download and print the form single-sided. Leave the signature box empty.

  5. Gather your documents and photo

Bring original evidence of US citizenship with a photocopy, a valid photo ID with a photocopy, and one 2x2 inch colour photo taken in the last six months.

  6. Apply in person and sign there

Take everything to a passport acceptance facility, or to a passport agency by appointment. Sign when the agent administers the oath, and pay the application fee to the State Department and the $35 execution fee to the facility.

What you need before you start

  • Original evidence of US citizenship, such as a birth certificate or previous passport, plus a front and back photocopy.
  • A valid photo ID, plus a front and back photocopy.
  • One 2x2 inch colour photo taken in the last six months.
  • For a child under 16: proof of parentage and both parents present, or Form DS-3053 or other evidence for an absent parent.
  • The fees from the State Department's passport fees page.

Can I sign Form DS-11 electronically?

No. The form itself says not to sign it until the authorized agent who administers the oath asks you to. You sign it in person, by hand, at the acceptance facility or passport agency. You can type your details in advance so the form is ready.

Common mistakes

  • Signing the form before the appointment.
  • Printing it double-sided.
  • Bringing a photocopy of the citizenship document instead of the original plus a copy.
  • For a child, only one parent attending without the second parent's consent form.
