Form DS-82 is the application for renewing an adult US passport by mail. It is only for people whose most recent passport meets the State Department's mail-renewal rules. Open the official form from the State Department in BunnyDoc's free Sign PDF tool straight from this page (nothing to download first), type your details into its fillable fields, and download it to print. Everything stays in your browser, so the file never leaves your device. Because the application is mailed on paper, the safest approach is to sign and date the printed form by hand.
How to fill in and sign Form DS-82
Check you can renew by mail
All of these must be true: you can send your most recent passport; it was never reported lost or stolen; it was issued in the last 15 years; it was issued when you were 16 or older; it is undamaged apart from normal wear; and it is in your current name, or you can document the name change. If not, you must apply in person with Form DS-11.
Fill in the form
Type your details into the PDF. The form says to print legibly in black ink only if filling by hand, not to correct or white out, and to start a new form if you make an error.
Print it correctly
travel.state.gov says to print on single-sided paper, as double-sided forms are not accepted. The form also says to print page one and page two on separate sheets.
Sign and date the application
The form says you must sign and date it in the designated area. Do this before mailing.
Add your photo, passport and payment
Staple one passport photo with four staples, vertically in the corners as close to the edges as possible, and don't bend it. Include your most recent passport, any name-change document, and a personal check or money order payable to "U.S. Department of State". Check the current fees on travel.state.gov.
Mail it to the right address
Use the address on the travel.state.gov mail-renewal page for your state and service type. Expedited applications have their own address and "EXPEDITE" written on the envelope.
Can I sign Form DS-82 electronically?
The State Department doesn't say whether a digitally added signature is accepted on the paper DS-82. The form says you must sign and date the application, and the renew-by-mail page says to sign and date it before mailing. Since it goes by post as a printed form, we recommend you print it single-sided and sign it by hand.
If you are eligible and want routine service, the State Department also offers online passport renewal, where the signing happens inside the government system.
