Form SS-5 is the application for a Social Security card: a first card, a replacement, or a corrected card after a name change. Many adults can now replace a card online, but others need the form. Open the official form from ssa.gov in BunnyDoc's free Sign PDF tool straight from this page, type into its fillable fields, add your signature and date, and download it. Everything runs in your browser, so the PDF never leaves your device. SSA asks for the form in black or blue ink, so print your copy before you take or mail it in.
