How to fill in and sign Form 3A
Tenant details (section 1)
Enter the full names of every tenant and the address of the property.
Earliest court date (section 2)
Work out the earliest date you can apply to court. It depends on the grounds you use; where you rely on several, wait for the longest notice period.
Latest court date (section 3)
This is 12 months after the notice is served, unless a Breathing Space extends it.
Your grounds and reasons (section 4)
Tick common reasons in 4.1 if you wish. In 4.2 copy the full legal wording of each ground from the official legal wording document, and in 4.3 explain why each ground applies.
Sign and give your details (section 5)
Sign, state whether you are the landlord, licensor or agent, date it and give your name, address and phone number. Joint landlords can each sign or one can sign for all.
Serve it and keep proof
Use the method in the tenancy agreement, or hand it over, deliver it to the property or send it by registered post. Email only counts if the agreement allows it. Keep a copy and record service, for example on form N215.

- Applying to court before the longest notice period has ended.
- Using Form 3A for social housing, where Form 3 applies.
- Relying on rent arrears grounds during a tenant's Breathing Space.
- Not keeping proof of service.
