The Verklaring Omtrent het Gedrag (VOG, certificate of conduct) shows that your criminal record has no relevance to a particular job or role in the Netherlands. Job applicants and volunteers who apply on paper use this Justis form, which they and the requesting organisation complete. Open the official form from justis.nl in BunnyDoc's free Sign PDF tool straight from this page, type into its fillable fields, add your signature and date, and download it. The PDF is handled in your browser and never leaves your device.
How to fill in and sign VOG
Applicant details (section 1.1)
Fill in your name, citizen service number (BSN), address, contact details, date and place of birth and nationality. Type it or use block capitals.
Applicant declaration (section 1.2)
Confirm the form is complete and truthful, and add the date, place and your name and/or signature.
Organisation details and purpose (sections 2.1 and 2.2)
The employer or organisation that needs the VOG enters its details and the purpose, such as a job or another purpose.
Screening profile (sections 2.3 and 2.4)
The organisation notes any special circumstances and chooses either a specific screening profile (2.4 a) or a general one (2.4 b), not both.
Organisation declaration (section 2.5)
A representative adds the date, place and signature and/or name. If no organisation can complete section 2, for example for a visa, fill in 2.1 to 2.4 yourself and attach a letter, email or web page showing why you need the VOG.
Hand it in at your municipality
Take the completed form to the Civil Affairs department of the municipality where you are registered, with valid ID, and pay the fee there.
What you need before you start
- Your BSN and a valid ID document.
- The organisation's details and the screening profile it needs.
- Payment for the fee at the municipality. The current form states €41.35.
Not registered in a Dutch municipality?
If you are not in the Personal Records Database (BRP), you cannot apply at a municipality. Use the separate VOG form for non-residents and email it to Justis.
Common mistakes
- Ticking both a specific and a general screening profile.
- Leaving the organisation name, purpose or screening profile blank, which puts the application on hold.
- Going to a municipality where you are not registered.
