How to fill in and sign Form 64-8
Check you need the paper form
HMRC asks you to use its online services or your business tax account where you can. Use the 64-8 only if you cannot. It is no longer accepted for limited authorisation, and it cannot be used for Making Tax Digital services.
Enter your details and your agent's details (page 1)
Give your name or business name, address and phone number, then the agent's business name, address and agent codes for Self Assessment or Corporation Tax if they have them.
Tick the taxes the agent can deal with (page 2)
Complete only the boxes that apply, such as PAYE for individuals (your National Insurance number), Self Assessment (NINO and UTR), Partnership, Trust, VAT, Construction Industry Scheme, tax credits, Corporation Tax or employer PAYE.
Sign and date the form
Sign and date page 1. A company's secretary or another responsible officer signs for a company, the responsible partner for a partnership, and a trustee for a trust. For joint tax credit claims, the joint claimant also signs.
Post pages 1 and 2 to HMRC
Send only pages 1 and 2 to National Insurance Contributions and Employer Office, HM Revenue and Customs, BX9 1AN, unless the form tells you to use a different office, for example for Corporation Tax only.
What you need before you start
- Your National Insurance number and Unique Taxpayer Reference (UTR).
- Your agent's name, address and agent codes. HMRC says the Self Assessment code is 6 characters, in the format 1111XX.
- Your company registration number and company UTR, VAT number or PAYE reference, if relevant.
- You must already be registered for Self Assessment before authorising an agent for it.
Common mistakes
- Writing outside the boxes or striking through unused boxes. HMRC asks you not to.
- Entering agent codes in the wrong format.
- Using the 64-8 for limited authorisation or Making Tax Digital, which HMRC no longer accepts.
- Adding a covering letter that is not needed.
- Sending page 3, which is guidance to keep.
