Form 64-8 lets you authorise an accountant or tax adviser to deal with HMRC for you. HMRC asks you to use an online service or your business tax account first, and to use the form when you can't. Open the official form from GOV.UK in BunnyDoc's free Sign PDF tool straight from this page (nothing to download first), type into its fillable fields, add your signature and date, and download it. The PDF is processed in your browser and never leaves your device.
How to fill in and sign Form 64-8
Check whether you can authorise online instead
GOV.UK says you can authorise an agent using an online service or the business tax account for many taxes. Use the 64-8 when you can't. HMRC no longer accepts the 64-8 for limited authorisation, and for trusts it does not give the agent online access.
Enter your details and your agent's details
Fill in your name (and the business, company or trust name), address and phone number. Your agent provides their business name, address, agent codes for Self Assessment and Corporation Tax, and client reference.
Tick the taxes you're authorising
Complete only the sections that apply, such as Self Assessment (National Insurance number and/or UTR), Corporation Tax (company number and UTR), employers' PAYE, CIS or VAT, with the reference numbers each section asks for.
Sign and date the declaration
Sign and date the form. For a company, the secretary or another responsible officer signs; for a partnership, the partner responsible for its tax affairs; for a trust, one or more trustees. For a joint tax credit claim, both claimants sign.
Post pages 1 and 2 to HMRC
Send only pages 1 and 2 and keep page 3. The form gives the address as National Insurance Contributions and Employer Office, HM Revenue and Customs, BX9 1AN, with exceptions (for example a Corporation Tax-only form goes to the office that deals with the company).
Can I sign Form 64-8 electronically?
HMRC doesn't say whether an electronic signature is accepted on the 64-8. The GOV.UK form page tells you to sign and date the form and post it to the address on the form. If you are unsure, ask your agent or HMRC, or authorise your agent online, which avoids the paper form.
What you need before you start
- Your National Insurance number and/or Unique Taxpayer Reference (UTR), or your company's registration number and UTR.
- Your agent's business details and agent codes (ask your agent).
- Other references for the taxes you're authorising, such as your PAYE or VAT number.
When not to use the 64-8
- To authorise an agent for Making Tax Digital for VAT: the form can't do that.
- For limited authorisation: HMRC says to write to them instead.
- During a compliance check: GOV.UK points to a different form (COMP1a).
