Form 4506-C lets an approved Income Verification Express Service (IVES) participant, usually a mortgage lender, get a transcript of your tax return straight from the IRS. Lenders normally fill in their own details and ask you to check and sign. Open the official form from irs.gov in BunnyDoc's free Sign PDF tool straight from this page, type into its fillable fields, add your signature and date, and download it. Everything runs in your browser, so the PDF never leaves your device.
How to fill in and sign Form 4506-C
Open the current form
Use the button on this page to open f4506c.pdf from irs.gov in Sign PDF. If your lender has sent you a pre-filled 4506-C, use theirs instead, because it already carries their IVES details.
Enter your name, TIN and addresses (lines 1a to 4)
Line 1a is your name and line 1b your SSN, ITIN or EIN. Lines 2a to 2c are for your spouse only if you want transcripts of a joint return. Line 3 is your current address and line 4 the address on your last return, if it was different.
Check the IVES participant details (lines 5a to 5d)
Line 5a holds the IVES participant's name, ID number, SOR mailbox ID and address. Line 5d must show the client company, such as your lender, and cannot be blank or "NA". These are normally filled in by the lender.
Choose the transcripts and years (lines 6 to 8)
Line 6 names one tax form, such as 1040, and the transcript type: return transcript, account transcript or record of account. Line 7 asks for wage and income transcripts, such as W-2 or 1099 data. Line 8 lists the year or period end dates.
Sign, date and tick the authority box
The taxpayer on line 1a signs, and the spouse on line 2a also signs if listed. Tick the box confirming you have authority to sign, or the IRS will not process the form. Lines 5a to 8 must be completed before you sign.
Return it to your lender
Send the signed form back to the lender or IVES participant, not to the IRS. They fax it to their assigned IRS IVES team, which must receive it within 120 days of your signature date.
What you need before you start
- Your name and SSN, ITIN or EIN exactly as on your tax return, and your spouse's details if the return was joint.
- Your current address and the address on your last return.
- The tax years the lender asked for.
- The IVES participant details from your lender, or the pre-filled form they sent you.
Can I sign Form 4506-C electronically?
Only if the IVES participant accepts it. The IRS page on IVES electronic signature requirements sets the rules that participants must follow, and the form itself says only participants who have opted in can accept electronic signatures. When it is signed electronically, the Electronic Signature box on the form must be marked.
Ask your lender how they want it signed. For background, see what an electronic signature is.
Common mistakes
- Signing before lines 5a to 8 are filled in.
- Leaving the box that confirms your authority to sign unticked.
- Leaving line 5d blank or writing "NA".
- Signing too early, so the IRS receives it more than 120 days after the signature date.
- A name or TIN that does not match the return being requested.
