How to fill in and sign Form 8821

  1. Open the current form

Use the button on this page to open f8821.pdf from irs.gov in Sign PDF.

  2. Your details (line 1)

Enter your name, taxpayer ID and address. Do not put the designee's details here, and note that this line does not update your address with the IRS.

  3. The designee (line 2)

Enter each designee's name, address and nine-digit CAF number, or "NONE" if they do not have one. You can tick a box to have copies of notices sent to them.

  4. The tax matters (line 3)

List the type of tax, the form number and the specific years or periods. General entries such as "all years" are not accepted, and future periods can go no more than three years past the year the IRS receives the form.

  5. Specific use and earlier authorizations (lines 4 and 5)

Tick line 4 for a one-off use the IRS will not record, such as a loan verification. Use line 5 to keep earlier Forms 8821 in force; otherwise they are revoked.

  6. Sign and submit (line 6)

Sign and date line 6. Submit online at IRS.gov/Submit8821, or fax or mail it to the number or address for your area in the instructions. An e-signed form must go through the online route.

What you need before you start

  • The tax forms and exact years or periods to cover.
  • The Form 8821 instructions for the fax numbers and addresses.

Can I sign Form 8821 electronically?

Yes, but only if it is submitted online. The IRS instructions say forms with an electronic signature must be submitted at IRS.gov/Submit8821, while forms sent by fax or mail need a handwritten signature. Accepted electronic signatures include a typed name, a digitized image of a handwritten signature or one drawn with a stylus or e-signature software.

When the person submitting signs you up remotely, they must have verified your identity. See what an electronic signature is for background.

Form 8821 or Form 2848?

Form 8821 only lets the designee see and receive your tax information. To have someone speak for you, argue your case or sign on your behalf before the IRS, you need Form 2848, Power of Attorney and Declaration of Representative.
