How to fill in and sign Form 4506-T
Enter your details (lines 1 to 4)
Line 1a is the name on the return (for a joint return, the name shown first) and 1b the first SSN, ITIN or EIN. Lines 2a and 2b are for a spouse on a joint return. Line 3 is your current address, and line 4 the address on your last return if it was different.
Line 5 is only a customer file number
Line 5 is optional and is not a third-party mailing address. The IRS no longer mails transcripts to third parties; lenders that need them directly use the IVES program (Form 4506-C).
Choose the transcript (lines 6 to 8)
Enter the tax form number (for example 1040) and tick 6a (return transcript), 6b (account transcript) or 6c (record of account). Use line 7 for a verification of non-filing, or line 8 for Form W-2, 1099, 1098 or 5498 information.
Enter the year or period (line 9)
Use the period end date in mm/dd/yyyy format, for example 12/31 of the tax year for a calendar-year Form 1040.
Tick the box, sign and date
You must check the box in the signature area confirming you are authorised to sign, or the form is returned unprocessed. For a joint return, at least one spouse signs. Add your phone number, signature and date.
Mail or fax it within 120 days
The IRS must receive the form within 120 days of the signature date. Send it to the address or fax number in the chart on page 2 of the form, which depends on the state you lived in.
Faster option: Get Transcript online
The form itself points out that you can get most transcripts faster through the IRS Get Transcript service or by calling 1-800-908-9946. Use the paper Form 4506-T when you can't use those routes or when someone has asked you for the signed form.
Can I sign Form 4506-T electronically?
The IRS doesn't say on the form whether an electronic signature is accepted for a mailed or faxed 4506-T. If a lender or school asked you for it, check what they accept. Lenders that use the IRS Income Verification Express Service usually ask for Form 4506-C instead, which has its own IVES e-signature requirements.
Common mistakes
- Leaving the authorisation box in the signature area unticked.
- Sending the form more than 120 days after signing it.
- Writing a lender's name and address on line 5, expecting the transcript to go to them.
- Listing more than one tax form number in a single request.
