Instructions
Official IRS page

Form 9465 asks the IRS to let you pay a tax bill in monthly instalments. If you owe $50,000 or less you can usually apply online at IRS.gov/OPA for a lower fee; the form is for everyone else and for people who prefer paper. Open the official form from irs.gov in BunnyDoc's free Sign PDF tool straight from this page, type into its fillable fields, add your signature and date, and download it. Everything runs in your browser, so the PDF never leaves your device.

How to fill in and sign Form 9465

  1. Open the current form

Use the button on this page to open f9465.pdf from irs.gov in Sign PDF.

  2. Your details (lines 1a to 4)

Enter your name and SSN, and your spouse's for a joint return. Tick line 1b if your address is new. Line 2 is only for a business that has closed. Add phone numbers on lines 3 and 4.

  3. Work out what you owe (lines 5 to 10)

Line 5 is the amount owed on the returns or notices, line 6 other balances, line 7 their total, line 8 any payment you are sending now and line 9 the balance. Line 10 is line 9 divided by 72.

  4. Choose your payment (lines 11a to 12)

Enter the monthly amount you can pay on line 11a, ideally at least the line 10 figure. Line 12 is the day of the month you will pay, no later than the 28th.

  5. Direct debit or payroll deduction (lines 13 and 14)

Give your bank routing and account numbers on line 13 to pay by direct debit, which lowers the fee. Line 14 is for payroll deduction, which needs Form 2159. Attach Form 433-F if the instructions require it.

  6. Sign and send it

Sign and date the form; both spouses sign for a joint return. Attach it to the front of your return if filing together, otherwise mail it to the IRS address for your state in the instructions.

What you need before you start

  • Your tax bill or notice, or the return showing the balance due.
  • Your SSN, and your spouse's for a joint return.
  • Your bank routing and account numbers if you want direct debit.
  • Form 433-F, Collection Information Statement, if you owe more than $50,000 or cannot pay at least the line 10 amount.

How much does a payment plan cost?

The IRS payment plans page lists the setup fees for long-term plans. Applying by mail, phone or in person costs $107 with direct debit and $178 for other payment methods. Applying online is cheaper. Low-income taxpayers have the direct debit fee waived and pay $43 for other methods, which may be reimbursed. Interest and penalties continue until the balance is paid.

Common mistakes

  • Using the paper form when you qualify for the cheaper online payment agreement.
  • Choosing a payment date after the 28th.
  • Leaving out Form 433-F when it is required.
  • Only one spouse signing after a joint return.
