How to fill in Form IND 7516
Situation, entry and TB (sections 1 to 3)
Say where the employee is now and whether they need a provisional residence permit (mvv), where they will collect it and when they will enter, and answer the tuberculosis questions.
Purpose and evidence (section 4)
Choose the purpose of residence, such as paid employment (370), and check the list of documents to enclose, for example the employment contract and passport copies.
Biometrics, sponsor and employee details (sections 5 to 7)
Read how the employee's photo, fingerprints and signature are taken, then enter the employer's details and the employee's personal details.
Collection and identification (sections 8 and 9)
Choose the IND desk where the residence permit will be collected and add the required copies of identity documents.
Sign by hand (section 10 and appendices)
Print the form, tick the declarations and sign section 10 in black or blue ink. Enclose the signed Declaration by sponsor (employment), the Employer's Declaration and the Antecedents Certificate signed by the employee.
Post it to the IND (section 11)
Send the application and copies (not originals) to IND, Postbus 5, 9560 AA Ter Apel. The IND will send a letter about the fee.
What you need before you start
- The signed employment contract.
- A copy of the employee's passport, including stamped pages.
- The appendices: Declaration by sponsor (employment), Employer's Declaration and Antecedents Certificate.
- The TB declaration, if the employee is not exempt.
- Sworn translations, and legalisation or an apostille where needed, for foreign-language documents.
Fees
The IND charges a fee for each application and sends a letter with the amount and how to pay after it receives the form. Fees are not refunded if the application is refused. The current amounts are on the IND fees page.
For 2026 the IND lists €423 for a first residence permit for paid employment.
Common mistakes
- Using an old version; the form footer shows the version, such as 7516-2026/2.
- Sending originals or stapling pages.
- Forgetting the employee's signed Antecedents Certificate.
- Posting an application that must be filed in person because it relies on an mvv exemption.
