Answer the questions (section 1)
Answer yes or no to each question about current or past prosecutions and sentences, serious crimes, entry bans in the EU, EEA or Switzerland, incorrect information in earlier residence procedures and past illegal stays.

If you answered yes to any question, explain the circumstances in the space provided.
Personal details (section 2)
Enter your V-number if you know it, your name, date of birth, and the place and date.
Print and sign by hand
Print both pages and sign section 2 with a black or dark blue pen.
Enclose it with the application
Give the signed appendix to the sponsor or include it with your own application form, and send it to the IND with the rest of the application.
What you need before you start
- Your full name and date of birth as in your passport.
- Your V-number, if the IND has given you one.
- Details of any convictions, entry bans or past stays without a residence permit.
Why honesty matters
The form warns that untruthful answers or failing to report are criminal offences that can lead to a sanction, and that your answers may affect your application or an existing residence permit. You must tell the IND within four weeks if your situation changes and your answers are no longer correct.

Where do I send form 7601?
You do not send it on its own. It is enclosed with the residence permit application, such as IND 7516 or IND 7518.
