How to fill in Form IND 7518
Situation and civic integration (sections 1 and 2)
Say where the family member is now and whether they need an mvv. Many family members must pass the civic integration exam abroad first, unless they are exempt.
Entry, TB and evidence (sections 3 to 6)
Choose where the mvv will be collected and the entry date, answer the TB questions, and choose the family relationship that applies, which decides the evidence to enclose.
Biometrics and personal details (sections 7 to 10)
Read the biometrics information, then fill in the sponsor's and the family member's details and any long-term mobility questions.
Collection and identification (sections 11 and 12)
Choose the IND desk for collecting the residence permit and enclose copies of the sponsor's ID and the family member's passport.
Sign by hand (section 13 and appendices)
Print and sign section 13 and the Declaration by sponsor in black or dark blue ink. Depending on your situation, also enclose the Declaration of relationship, Declaration of civil status, proof of income and the family member's Antecedents Certificate.
Post it to the IND (section 14)
Send everything as A4 copies, without staples, to IND, Postbus 16, 9560 AA Ter Apel, the address for family purposes.
What you need before you start

- Proof of the family relationship, such as a birth or marriage certificate.
- Proof of income, unless an exemption applies.
- The civic integration exam result or proof of exemption.
- The signed appendices for your situation.
Fees
The IND charges a fee for each application and sends a letter with the amount and how to pay after it receives the form. Fees are not refunded if the application is refused. The current amounts are on the IND fees page.
For 2026 the IND lists €85 for a child under 18 joining a parent and €254 for a partner.
Common mistakes
- Sending originals; the IND asks for copies only.
- Forgetting the signed Declaration by sponsor.
- Using this form when the family member is already in the Netherlands and must file the foreign national's form.
- Applying for the mvv more than a year after the civic integration exam result was issued.
