How to fill in and sign Form W-7
Open the current form
Use the button on this page to open fw7.pdf from irs.gov in Sign PDF. Tick whether this is a new ITIN or a renewal.
Choose your reason (boxes a to h)
Tick the box that fits, for example b for a nonresident alien filing a US return, d for a dependant of a US citizen or resident, or e for a spouse. Box a and box h need extra details such as the treaty country and article.
Enter your details (lines 1 to 6)
Give your name and name at birth, mailing and foreign addresses, date and place of birth, and details of your identity document on line 6, plus any previous ITIN.
Sign and date
Sign the form yourself. For a dependant under 18 a parent or court-appointed guardian may sign; anyone else signing needs a Form 2848 power of attorney.
Attach your return and documents
Attach your federal tax return unless an exception applies, and your original identity documents or copies certified by the issuing agency. A valid passport alone is usually enough.
Send or take it to the IRS
Mail it to the IRS ITIN Operation in Austin, Texas, take it to an IRS Taxpayer Assistance Center that verifies documents, or use a Certifying Acceptance Agent who can check your documents and return them on the spot.
What you need before you start
- A valid passport, or at least two other documents proving identity and foreign status, as originals or certified copies.
- The federal tax return the ITIN is needed for, unless you qualify for an exception.
- Treaty details if you are claiming treaty benefits.
- The Form W-7 instructions.
Common mistakes
- Sending notarized copies instead of originals or copies certified by the issuing agency.
- Leaving off the tax return when no exception applies.
- Mailing it to the address for your tax return instead of the ITIN Operation address.
- Applying for an ITIN when you are eligible for an SSN.
Keeping your documents safe
Original documents sent by mail are returned, but it takes time. If you do not want to post your passport, a Certifying Acceptance Agent or an IRS Taxpayer Assistance Center can verify your documents in person and hand them back.
